US Treasury BOI Questionnaire

The CTA law exempts 23 types of businesses, including accounting firms, banks, charitable entities, and large operating companies that meet certain requirements. Please answer the following questions to determine if you must file the BOI report for your business entity.
Does your business have less than 20 full time Employees? Yes or No

Does your business have an operating presence in the United States? Yes or No

Did your business have more than $5m in revenue, gross receipts, or sales? Yes or No
Does your business entity fall under any of the following categories? Yes or No
• Bank or Credit Union • Securities Broker or Dealer
• Accounting firm • Large operating Company
• Money Services Business • Pooled Investment Vehicle
• A company regulated by the SEC • Inactive Entities
• Public Utility Company • Government Authorities
• Public utility company • Financial Market Utilities
• Tax Exempt Entity • Nonprofit Charities
Does your business entity own, hold or have interest in any assets, real or
personal in the United States or any foreign jurisdiction, including any
interest in any Limited Liability Company, Corporation or similar entity? Yes or No
If you have answered "No" to all of the above questions you are exempt from filing.

If you answered "yes" to any of the above questions you are required to file.
Most US small businesses and holding companies are required to file.
